Pay: $75,000.00 - $88,000.00 per year

PRINCIPAL DUTIES AND RESPONSIBILITIES
- Develop personal care service plans to ensure consistent wellness of each resident. Monitor the wellness and health of the residents (weight, blood pressure, nutrition, etc.), consistent with the service plan. Complete ongoing assessment of residents and recommends service plan changes to insure quality.
- Assist with the implementation of the self-administered medication management program. Collaborate with pharmacy to ensure accurate medication delivery and up-to-date resident medication regime. Monitor the program and educate the staff as needed.
- Administer medication to residents as needed, according to the E.O.E.A. regulations.
- Collaborate and maintain routine communication with families and community agencies, i.e. VNA to ensure quality resident services.
- Role models geriatric care techniques to staff and educate as needed.
- Monitor written staff documentation, including daily documentation sheets, logbook, and self-administered medication sheets for accuracy and completeness.
- Participate in meetings to discuss needs and requirements of each resident and takes appropriate action as needed and directed to promote quality resident care.
- Responsible for promoting an environment that results in resident satisfaction by exhibiting a positive attitude and timely response to resident needs and requests. Exhibit creativity in approach and implementation.
- Follow the department and Orchard Hill Assisted Living, policies and procedures and federal, state and local laws and regulations, where applicable
- All other duties as required and consistent with the education and skills required for this position.
EDUCATION/EXPERIENCE/LICENSURE-CERTIFICATION
EDUCATION: Graduate of an approved LPN/RN program.
PREVIOUS EXPERIENCE: Minimum 1 to 2 years experience working as an LPN or RN Previous experience working with the elderly and/or disabled in a paid or volunteer position highly desirable.
LICENSE: Current state license as a LPN or RN Eligible for CPR certification and ongoing re-certification.
SKILLS AND ABILITIES
- Demonstrated excellence in understanding and utilizing nursing assessment skills.
- Familiarity and experience using medical related equipment, including but not limited to blood pressure kit, personal protective equipment (gloves, mask, goggles, etc.), thermometer and transfer equipment.
- Excellent verbal communication skills and the ability to relate in a positive and professional way to a diverse resident and employee population.
- Ability to be flexible and work in an environment that promotes teamwork and collaboration.
- Ability to maintain a high level of confidentiality regarding residents, employees, staff and the community.
- Demonstrated ability to set priorities, complete assignments in a timely manner while managing multiple duties and responsibilities.
- Working knowledge of all office equipment, including but not limited to calculator, computer, copier and fax machine.
PHYSICAL REQUIREMENTS
- Physically able to move at least 50 lbs. without assistance.
- Physically able to bend, reach and work in small areas.
- Physically able to push and pull equipment and furnishings.
- Physically able to stand for long periods of time.
